The Shelter Plus Care program is an integral part of the continuum of care for the homeless in the District of Columbia. The program provides case management and supportive services five days a week to homeless families recovering from substance abuse, have a diagnosis of HIV/AIDS and are receiving rental assistance for permanent housing from the The Community Partnershipfor the Prevention of Homelessness. Case management and supportive services include supportive services may include but are not limited to:
- Outreach
- Case management
- Life skills (outside of case management)
- Alcohol/substance abuse services
- Mental health services
- HIV/AIDS related services
- Other health care services
- Financial management
- Education assistance
- Employment assistance
- Child care
- Transportation
